CHCPE
Connecticut Home Care Program for Elders
A state program that helps eligible older adults receive care at home instead of moving into a nursing home. To qualify for the Connecticut Home Care Program for Elders (CHCPE), applicants must be Connecticut residents aged 65 or older, require hands-on help with daily activities, and meet the state's income and asset financial limits. We help you apply, and once you are enrolled, we help keep your paperwork up to date.

Care for a loved one in a comfortable, family-style home. We guide families through registration and enrollment, one step at a time.

2. We call you back. If you filled out the form on your own, we will ask a few more questions to understand, and explain, which program may fit.
3. We handle the paperwork with you. Once everything goes through and you are approved, your care with St. Daniel's Companions can begin, with a caregiver matched to you.
